Merge protections honor GitHub's "require approval of most recent push"

When your branch protection enables GitHub’s “Require approval of the most recent push” option, Mergify now automatically adds a matching condition to your merge protections. A pull request won’t merge until its latest push has been approved by someone other than the author, keeping Mergify aligned with GitHub’s own enforcement.
